Written Paper Records

Write paper records in ink.

Sign and date each entry.

Make alterations by placing a single line through the error and date the amendment.

As patients are entitled to have access to their records (see Ethical Practice),avoid using any derogatory comments about the patient. Personal comments should be avoided.

Use abbreviations that are consistent and understood by all users.

Ensure records are complete, clear accurate and legible.
